When to Look Beneath the Surface

If your baby or toddler isn’t sleeping well, and you’ve passed the "it's just a phase” part of dealing with it (sometimes it truly is just a phase!), it may be time to look at the bigger picture and rule out some common sleep red flags. 

One of the biggest things I want families to understand is this:

Not all sleep challenges are behavioral.

Before we focus on schedules, routines, or trying to “fix” sleep, it’s important to ask: Could something deeper be going on?

Below are five common sleep red flags I look for as a certified baby and toddler sleep specialist and consultant.

1. Hourly Waking in Babies and Toddlers

Waking during the night is completely normal. Developmental leaps, teething, illness, and travel can all temporarily disrupt sleep.

But if your child is waking every hour, consistently, it may be a sign of an underlying issue.

Hourly waking can sometimes point to:

  • Physical discomfort

  • Restlessness

  • Feeding imbalances

  • Airway or breathing concerns

If it feels persistent or intense, it’s worth taking a closer look before assuming it’s purely behavioral.

2. Airway Issues and Sleep

Sleep quality is directly connected to how well your child can breathe.

If breathing is compromised during sleep, the body will often wake frequently to protect the airway.

Signs of airway-related sleep issues:

  • Mouth breathing

  • Snoring

  • Chronic congestion

  • Pauses in breathing

  • Gasping, choking, or coughing at night

  • Enlarged tonsils or adenoids

If you’re noticing these symptoms regularly, consider discussing them with your pediatrician or an ENT specialist.

3. Feeding Issues That Impact Sleep

Feeding and sleep go hand in hand, especially in infancy.

If feeding is uncomfortable or inefficient, it can lead to:

  • Increased night wakings

  • Digestive discomfort

  • Difficulty settling

Signs of feeding challenges:

  • Clicking while feeding

  • Milk leaking from the mouth

  • Gulping or choking

  • Short, frequent feeds

  • Frequent spit-up or vomiting

  • Gas or fussiness after feeds

These may be linked to oral ties, reflux, or food sensitivities.

When feeding improves, sleep often follows.

4. Low Iron and Sleep in Children

Iron deficiency is one of the most common (and overlooked) causes of sleep disruption in young children.

Low ferritin levels can contribute to:

  • Difficulty falling asleep

  • Restless sleep

  • Frequent night wakings

  • Dark circles under the eyes

  • Daytime fatigue

If this is a concern, ask your pediatrician about testing ferritin levels (not just hemoglobin). If your baby is primarily breastfeed, it’s important to check your levels too! 

5. Eczema and Night Wakings

Eczema can significantly impact sleep due to discomfort and itchiness, especially at night.

While topical treatments can help manage symptoms, it’s also important to consider potential root causes.

Common eczema triggers may include:

  • Food sensitivities

  • Environmental irritants

  • Skin barrier issues

If your child’s eczema seems to worsen sleep, it’s worth exploring further with your care provider.

Final Thoughts: Sleep Is Holistic

Sometimes, it means we need to look beneath the surface.
